We Provide

  • Support in following up approval and assignment of your Home Care Packages
  • The expertise you need to make the most out of your Home Care Package.

Ongoing Support

  • To dedicate and respond to your changing needs.
  • To choose your provider and we will liaise with them.
  • To ensure all service providers are suitably credentialled.

Your Care Partner

  • Will provide initial and ongoing support and assessment of your health and social needs to enable you to do the things you want to.
  • Will create personalised home visits and continue to monitor your health and wellbeing.
  • Will work with you as your needs change and maintain communication with your family to review your services.
  • Will provide guidance and simplify the process to ensure all needs are met.

Accounts

  • Ensure you are getting the most value out of your package
  • Provide a monthly statement showing the balance of your package funds.
  • No daily fee or exit fees
