General Practice Services

We are a friendly, professional and experienced General Practice team comprising of General Practitioners, Practices Nurses (both RN and EENs) and Clinical Psychologist. We also closely work alongside Geriatricians and Allied Health Professionals. The team at New Aged Care provides specialised medical care to you in your Aged Care Facility. We are interested in your needs and are committed to being part of your care team.


Our exceptional General Practice team model for Aged Care Facilities has been used with great success for many years. Residents, families and facility staff are delighted with the quality and accessibility of our services. We Provide

  • Regular weekly attendances by our team consisting of GPs, Nurses and Allied Health Professionals.
  • Quality and accessible services specialised to meet your needs.
  • Additional visits and/or phone advice is also provided for any concerns that may arise in-between visits.
  • Support in the transition to Residential Aged Care.
  • Family conferences and/or phone advice to families are provided as required.
  • Preparation of clear care plans, regular comprehensive medical assessments, and advance care plans to facilitate the best possible quality of care tailored to your needs and wishes.
  • Referral and organisation of Specialist and Allied Health services as required.
  • Our GP Services are Bulk-Billed

Your GP & Nurse

  • Will establish a meaningful connection with all patients to ensure the transition into Residential Care is made easy.
  • Will work with you as your needs change and maintain communication with family.
  • Will provide guidance and simplify the process to ensure all needs are met.

Ongoing Support

  • We offer a multidisciplinary team approach to your care.
  • We make clear treatment decisions and document these so that all staff can consistently manage the resident's health care.
  • Will assist families to understand all medical and clinical information as it arises.

We are supportive and understand the many pressures that can arise from having a loved one at a Residential Aged Care Facility. We recognise in some circumstances moving into aged care can be difficult but we work to support you during this transition.
